Submission Requirements Please consider the following guidelines when submitting applications to Synergy Coverage Solutions: Completed Accord (Forms 125 and 130) application, including insured’s FEIN (Federal Employer Identification Number) Loss runs for current year and three prior years, and a copy of the experience mod worksheet Detailed description of account on summary page or application, Financials, engineering reports, safety manual, etc. Target premium Binding Requirements For a policy to be bound and effective, the following qualifications must be met: Satisfactory payment arrangements or payment plan must be made (view more information about our billing and finance options) A completed and signed application submitted by a licensed agent or broker Completed list of officers, including names, titles and exclusions (if applicable)
